OSSB Foundation Grant Application

The Ohio State School for the Blind Parent Teacher Staff Foundation (OSSBPTSF) provides monetary support to benefit school-age children who are blind or visually impaired, and receiving services from the Ohio State School for the Blind (OSSB).

The OSSBPTSF Board of Directors determines whether to award grants at the Board meetings. The OSSBPTSF Board meetings for the 2025-26 school year will be held:

September 9, 2025
November 18th, 2025
January 20, 2026
March 10, 2026
May 19, 2026
July 21, 2026

Grants or proposals must be submitted two weeks prior to an OSSB PTSF Board meeting, and must include the following information:

Please provide the following information regarding the proposed project:

A. Brief statement regarding the organization, department, or individual requesting funds.

B. Statement of the program or project to be funded.

C. The amount requested

D. The proposed use of the funds if awarded.

E. Payment method & contact info if awarded

F. Other funds or supports for proposed project from other organizations.

G. The evaluation mechanism that will be used to evaluate the success of the project.

H. What impact will it have on the students or school project that it will be serving?
